目的:探究少精子症对基于人工智能(AI)识别读取的荧光法与基于流式细胞仪检测的精子染色质结构测定法(SCSA)检测精子DNA碎片指数(DFI)的影响。方法:收集201例男性患者精液样本,其中50例无精子症样本作为阴性对照,其余样本按照精子浓度不同分为少精子症组(0×106/ml<精子浓度<15×106/ml)和正常组(≥15×106/ml),分别为90例和61例,再将少精子症组细分为轻度少精子症组(10×106/ml≤精子浓度<15×106/ml)、中度少精子症组(5×106/ml≤精子浓度<10×106/ml)以及重度/极度少精子症组(0×106/ml<精子浓度<5×106/ml)共3组,每组各30例。比较AI荧光法与传统流式法检测各组精子DFI的差异。结果:AI荧光法与流式法的DFI值,在正常组和轻度少精子症组无统计学差异(P>0.05),在无精子症组、中度少精子症组和重度/极度少精子症组存在显著统计学差异(P<0.01),且AI荧光法的DFI值明显低于流式法;在无精子症组,流式法出现非0结果的比例为100%,AI荧光法出现非0结果的比例为8%,两种方法存在显著统计学差异(P<0.01)。对于正常组和轻度少精子症组样本,流式法与AI荧光法检测DFI结果的相关性较好(R2=0.747 0,R2=0.718 0),对于少精子症组全样本、中度少精子症组和重度/极度少精子症组样本,流式法与AI荧光法检测DFI结果的相关性较差(R2=0.309 2,R2=0.355 8,R2=0.214 7)。结论:AI荧光法检测精子DFI的特异性可能优于流式法,少精子症的DFI检测可能用AI荧光法更合适;当精子浓度≥10×106/ml时,基于流式法和AI荧光法检测的DFI结果一致性较好;当精子浓度>0×106/ml且<10×106/ml时,基于流式法和AI荧光法检测的DFI结果准确性可能会受影响。
